Continuum of Care Department
The hallmark of our Continuum of Care Department is the proactive management of patient treatment for the best possible healthcare outcomes. With the depth of services offered through New York Downtown Hospital, it is important that all phases of treatment are successfully coordinated. Our Continuum of Care staff ensures that members of the inpatient healthcare team are working in close collaboration and communicating effectively during the patient’s stay and after discharge. The department actively follows the progress of inpatients, even as they move into rehabilitation or follow-up treatment. Department Case Managers, who are also registered nurses, guide patient care through a full range of health services and treatment levels. These managers are the integral link between patient, providers, insurance companies and the community. They also streamline communication between patients, families and physicians. The Social Workers with our Continuum of Care Department in New York City assist patients and their families as they cope with the social and psychological problems that may result from an illness. These dedicated staff members provide support, counseling, and crisis intervention, as well as referrals to community agencies. When needed, they assist families in selecting appropriate nursing homes or rehabilitation facilities for continuing care.
The Downtown Hospital offers superior patient support and clinical expertise at every stage of treatment. Continuum of Care professionals are here to assist discharged patients who may need additional medical services, whether in the form of compassionate New York hospice care or physical therapy. With our continuity of care, patients and their families are informed participants in the delivery of our interdisciplinary services.
